Not all of the services that we offer are covered by the NHS. Here is a list of the private services that are available at the Masharani Practice.  As these services are private they incur a charge, for details please contact the surgery.

CERTIFICATES AND FORMS:

• Private Sick Note – please note we do not provide a free sick note for the first week of your illness. You are expected to self-certify for your illness, but you are able to purchase a private sick note if you wish.
• Passport Form/Photograph
• Driving Licence Photograph
• Freedom From Infection Certificate
• Provident Association Claim Form (BUPA/PPP)
• School Fees Insurance Form
• School Letters
• Sickness and Accident Insurance Benefit Claim Form
• Fitness to Exercise – Health Club Etc
• Letter for the Housing Department
• Health Declaration Form
• Child-Minding Forms

MEDICAL EXAMINATIONS AND REPORTS:

• Pre-employment Medicals for HGV/Sports/Taxi Driving
• Power of Attorney Forms
• Vaccination Certificate
• Holiday Cancellation Certificate
• Fitness to Travel Certificate
• Certificate to Carry Medication/Needles on Board Aircraft (Hand Luggage)

PLEASE NOTE THAT ALL FEES WILL NEED TO BE PAID IN ADVANCE AND IN CASH PLEASE – WE NO LONGER ACCEPT PAYMENT BY CHEQUE AND DO NOT HAVE THE FACILITIES TO ACCEPT CARD PAYMENTS.  THANK YOU.
