Before filling out a registration form, check you are in our catchment area, we only accept patients within our practice boundary.

Registering with the surgery

To permanently register with the surgery:

• Fill out a registration form – once completed this form can either be handed in at reception or scanned and emailed to themasharani.practice@nhs.net – please mark the email “New Patient Registration”.

• Visit reception from 10am to 6pm and collect a paper copy of the registration form, complete it and hand it back in at reception or post to the surgery address at the top of the page.

• We are using a new online service called “Register with a GP Service that makes it easy to register with this GP surgery
  • Just fill in this quick online form to start the process https://gp-registration.nhs.uk/C82611
  • The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Temporary registration  A temporary patient is someone who needs to register with the practice temporarily who cannot access their own GP Practice. You can register as a temporary resident with a GP in England if you are in the area for longer than 24 hours but less than three months. 

We will also see you as an emergency if you are passing through ( e.g. staying at one of the local hotels) and need emergency medical attention.
